Renewal certification for ACM (fee $15.00): Please complete this form and enclose it with your five stubs. (Xerox copies are acceptable, providing both front and back are copied.) (SMT and Jazz programs may not count toward certification.)

Your eligibility depends on your having fulfilled the following requirements:
1. Initial certification: During the past five years, 25 students have passed national or international programs, each program receiving a minimum of 14 more C's than A's.
2. Certification renewal: During the current audition year, five students have passed national or international programs, each program receiving a minimum of 14 more C's than A's.
